IMPORTANT: You must get the FORM NOTARIZED. We DO NOT have a notary available at the track. Most banks provide free notary services to their account holders.
NOTES: Forms are good for 1 year. In the "Event Dates" section of the form please write in "Jan 2012 - Dec 2012".
